Device Agreement Form — Please complete this form if your student will be borrowing a laptop or device from B.E.A.M. Foundation.

Device Loan Terms

Thank you for registering for Summer Tech Accelerator! If your student will be borrowing a device from B.E.A.M. Foundation, please review the terms below and complete the agreement form.

Device Use: The loaned device is for educational use only during Summer Tech Accelerator sessions (July 11–22, 2026) and related coursework.

Parent/Guardian Responsibilities:

Supervise device use by student
Keep device in safe condition
Return device in the same condition it was received
Report any damage immediately to B.E.A.M. staff

Return Deadline: All devices must be returned by Wednesday, July 23, 2026 at the final session.

Pickup/Return: DoGreater Charlotte, 2400 Greenland Avenue, Charlotte, NC 28208

Liability: Parent/Guardian assumes responsibility for normal wear and tear. Damage beyond normal use may result in repair costs to be determined by B.E.A.M. Foundation.
